"To qualify for the Equipment Distribution Program, an audiologist or physician must certify that the applicant needs the equipment due to his or her degree of hearing loss or speech impairment. Qualified applicants must have an annual household income no greater than $45,000. Only one of each device will be distributed per household in a five- year period."
